Our office would like to express the importance of knowing your insurance. Insurance is rapidly changing, and to to this, we advise that you review your insurance policy prior to all appointments. This includes , but is not limited to:

Copays for all appointment types

Physicals and Well Child coverage

Deductibles

If you do not know what your copay is, the office will collect $40.00. All copays will be collected at the time of check-in.
